Application
Form CC-1411: Application for Change of Name (PDF) is to be used by adult applicants only. If you wish to change the name of a person under the age of 18, you may need to contact an attorney.
You may print the application off for yourself or visit the Circuit Court Clerk's Office to receive a paper copy.
Filing
Pursuant to Code of Virginia § 8.01-217, the Petition for Name Change must be filed in the Circuit Court of the county/city where the applicant resides. The application is to be made under oath, meaning that it must be acknowledged and signed in the presence of a Notary Public or Deputy Clerk.
The application must also be completely filled out prior to being filed in the Clerk's Office. The Clerk's Office cannot assist you in filling out the form. Additionally, there is a $39.00 filing fee.
